Haemolysis Elevated Liver enzymes Low Platelets
A rare but serious condition that it part of the pre-eclampsia spectrum. It can deteriorate very quickly. There is a 10-60% risk of perinatal mortality and a 1% risk of maternal mortality (often post partum to pulmonary oedema). There is a high risk of re-occurrence in subsequent pregnancies.

Signs and Symptoms

  • Epigastric/RUQ pain and tenderness
  • Nausea and vomiting
  • Hypertension
  • Proteinuria
  • Raised LFTs (tenderness may precede this)
  • Low platelet count


  • Stabilise mother with antihypertensives
  • Correct coagulopathy with blood products
  • Induce delivery
  • Give steroids, pre and post partum
