Bronchiectasis

Irreversible dilatation of one or more bronchi, with chronic airway inflammation; due to bronchial wall damage from infection and inflammation.

Causes

A SICK AIRWAY:

  • Airway lesion, chronic obstruction
  • Sequestration
  • Infection, inflamation- H.influenzae, strep pneumonia, staph aureus, pseudomonas aeruginosa measles, pertussis, bronchiolitis, pneumonia, TB, HIV
  • Cystic fibrosis
  • Kartagners syndrome
  • Allergic aspergilliosis
  • Immunodeficiencies (hypogammaglobinaemia, myeloma, lymphoma)
  • Reflux
  • William Campbell syndrome
  • Aspiration
  • Yellow nail syndrome/ Young syndrome

Symptoms

  • Persistent cough + lots of sputum (purulent).
  • Intermittent haemoptysis
  • SOB
  • Chest pain

Signs

  • Coarse inspiratory crackles
  • Clubbing
  • Wheeze

Management

  • Postural drainage, twice a day
  • Antibiotics; as needed
  • Bronchodilators; for coexisting asthma, COPD, CF
  • Corticosteroids
